【摘要】 目的 探讨survivin和p27kip1蛋白在结肠癌中的表达及其相互关系。方法 应用免疫组织化学方法检测83例结肠癌标本和10例正常结肠黏膜组织中survivin、p27kip1的表达。分析survivin、p27kip1的表达与临床病理参数的关系。结果 survivin在结肠癌组织中阳性表达率83.1％（69/83）明显高于在正常结肠黏膜组织中的0.0%（0/10）(p<0.01)。结肠癌中survivin的表达与临床病理参数无关（p>0.05）。p27kip1在结肠癌组织中阳性表达率41.0%（34/83）明显低于在正常结肠黏膜组织中的90%（9/10）（p<0.01）。远处转移组p27kip1阳性率(16.7%，3/18)明显低于远处无转移组(47.7%，31/65)(p<0.05)。结肠癌p27kip1表达与组织分化、肠壁浸润深度、dukes分期有关（p<0.05）。结肠癌组织中survivin表达与p27kip1表达未见明显相关性（p>0.05）。结论 survivin和p27kip1蛋白在结肠癌组织中存在异常表达，提示两者在结肠癌的发生发展中起着重要的作用。结肠癌中survivin的表达与p27kip1蛋白的表达无相关性。

[abstract] objective to investigate the expression of survivin and its correlation with expression of p27kip1 protein in colon carcinoma. methods expression of survivin and p27kip1 protein was investigated by immunohistochemistry in 10 cases of normal colon tissues and 83 cases of colon cancer tissues. the relationship between survivin expression and clinicopathological parameters as well as p27kip1 expression was analyzed. results the positive expression rate of survivin was significantly higher in colon cancer tissue (83.1%, 69/83) than in normal colon tissue (0.0%, 0/10) (p<0.01). expressions of survivin were not related to the clinicopathological parameters (p>0.05). the positive expression rate of p27kip1 was significantly lower in colon cancer tissue (41.0%, 34/83) than in normal colon tissue (90.0%, 9/10) (p<0.01). the positive expression rate of p27kip1 in colon cancer was significantly lower in distant metastasis group (16.7%, 3/18) than in nondistant metastasis group (47.7%, 31/65) (p<0.05). the expression of p27kip1 was significantly correlated with histological differentiation, invasion depth and dukes stages (p<0.05). there was not correlation between the expression of survivin and p27kip1 (r=0.096，p>0.05).conclusion the abnormal expression of survivin and p27kip1 protein indicates that survivin and p27kip1 may play an important role in onset and progression of colon cancer. no correlation is found between survivin and p27kip1 expression in colon cancer.

结直肠癌是最常见的消化道癌瘤之一,严重威胁人类健康。WWW.11665.cOmsurvivin是近来发现的抑制凋亡基因家族（iaps）的新成员。p27kip1是一个周期素依赖性激酶抑制蛋白，是细胞周期g1期向s期转换的负向调控因子。文献未见关于结肠癌中survivin与p27kip1相关性的研究报道，两者在结肠癌中的关系尚未明确。本研究采用免疫组化法检测了结肠癌中survivin和p27kip1的表达，探讨两者在结肠癌中的表达特点、意义及其相关性。
1 材料与方法
1.1 材料 肿瘤标本取自2007年9月至2008年10月在广西医科大学第一附属医院结直肠肛门外科进行手术治疗并经病理证实的结肠癌标本83例，其中男52例，女31例，年龄20～82岁，平均54岁。dukes’a期9例，b期16例，c期31例，d期27例。有淋巴结转移者50例,送检淋巴结证实至少1枚有转移，无淋巴结转移者33例，至少送检12枚淋巴结。另选择10例正常结肠黏膜组织作为对照。
1.2 方法 采用superpic ture tmpolymer二步法免疫组化法[1]。survivin广谱抗人多克隆抗体，p27kip1鼠抗人单克隆抗体，hrp polymer（二抗）均购自福州迈新生物技术开发有限公司，superpic ture tmpolymer 试剂盒购于天津津脉基因测绘技术有限公司。对照组以pbs代替一抗，已知阳性的胰腺癌切片作为survivin的阳性对照片，已知阳性的肾盂肾炎切片作为p27kip1的阳性对照片。
1.3 结果判定 survivin多定位于细胞浆；p27kip1多定位于细胞核，细胞浆内也有部分表达。参照kawasaki[2]的半定量积分法判断结果。分别由两位病理医师在独立情况下对免疫组化结果作出判断,然后核对所得结果,获得共识。本研究在进行数据处理时，survivin、p27kip1以阴性(-)作为阴性组，以弱阳性(+)、中度阳性(++)，强阳性(+++)合并作为阳性组。
1.4 统计学分析 应用spss13.0统计软件包进行统计学处理。不同组间阳性率差异用χ2检验，采用spearman等级相关分析。
2 结 果
2.1 survivin和p27kip1在结肠癌与结肠正常黏膜中的表达 survivin在结肠癌组织中阳性表达率83.1％（69/83）明显高于在正常结肠黏膜组织中的0.0%（0/10）(χ2=28.018，p<0.01)。p27kip1在结肠癌组织中阳性表达率41.0%（34/83）明显低于在正常结肠黏膜组织中的90%（9/10）（χ2=6.773，p<0.01）。
2.2 survivin和p27kip1与结肠癌病理参数的关系 survivin的阳性率在各病理参数间均无差别（p>0.05)，见表1。p27kip1的阳性率与年龄、性别、肿瘤部位、大体分型、肿瘤直径、淋巴结转移等无关（p>0.05）；p27kip1与组织分化、肠壁浸润深度、远处转移、dukes分期有关（p<0.05）；远处转移组p27kip1阳性率(16.7%，3/18)明显低于无远处转移组(47.7%，31/65)（χ2=5.611，p<0.05），见表2。

3 讨 论
结直肠癌准确的分期有赖于从切除标本中准确获取和检查淋巴结，美国癌症联合会和美国病理学家协会建议至少需检出12枚淋巴结才能准确判断为ⅱ期结直肠癌[3]。在本研究中，对于a、b期的病例送检淋巴结数目最少要达到12枚；d期中无淋巴结转移者，也要求至少送检12枚淋巴结；c期、d期有淋巴结转移者，送检淋巴结证实至少1枚有转移。因此，本组病例的淋巴结是否有转移及分期是较明确的，为研究survivin和p27kip1表达与淋巴结转移及分期的关系提供了良好的病例基础。
survivin是新近发现的一种抗凋亡基因，属于抑制凋亡基因家族（iaps）的新成员。近年来研究表明，survivin是迄今为止发现的最强的凋亡抑制因子，其抗凋亡功能在于直接抑制了半胱氨酸蛋白酶caspase3和caspase7的活性，阻断细胞的凋亡过程[4]。近几年来的研究结果显示，survivin除可在胚胎组织和成人的正常胸腺组织中表达外，未发现其它正常成人组织中有survivin表达，而在多数肿瘤组织中均有survivin的高表达，成人已分化组织则表达下调或不表达。本研究显示survivin在结肠癌组织中阳性表达率(83.1％)明显高于正常结肠黏膜组织（0.0%），提示survivin可能参与了结肠癌的发生。本研究显示survivin蛋白的表达与临床病理特征间无相关性，与ponnelle[5]的报道一致。survivin的表达与临床病理指数无相关性，这可能与survivin的表达是结直肠癌发生过程的早期分子事件有关。因此survivin蛋白检测在结肠癌早期诊断与结肠肿瘤的良恶性鉴别诊断方面有着一定的临床意义。
p27kip1是新近发现的一种肿瘤抑制基因，是细胞周期素依赖性激酶抑制蛋白(cyclin depentent kinase inhibitors, cdki)家族新成员之一。p27kip1与传统的抑癌基因不同，尽管它在多种恶性肿瘤中表达，如乳腺癌、结直肠癌、肺癌、恶性淋巴瘤、膀胱癌和甲状腺癌等，但极少发现有纯合子缺失或突变，且它的表达往往是翻译后调节[6]。本研究中发现p27kip1在结肠癌中的表达明显低于在结肠正常黏膜组织的表达，这与loda[7]的报道一致，提示p27kip1可能参与了结肠癌的发生。本研究中，结肠癌中p27kip1的表达与分化程度、dukes分期、浸润深度、远处转移有关，与文献报道[8]的大体一致，提示p27kip1与结肠癌的发展有关。p27kip1蛋白表达降低更易发生深层浸润和远处转移，提示p27kip1低表达可能与结肠癌预后不良相关。
恶性肿瘤的发生、发展不仅表现为细胞增殖过强，而且与细胞凋亡减弱密切相关；细胞的癌变是多个基因表达共同协作的结果。survivin属于正调控因子，能够对抗g2/m期细胞凋亡的诱导作用，促进通过细胞调定点，从而促进肿瘤的发生发展。p27kip1作为一个周期素依赖性激酶抑制蛋白，属于负调控因子，具有阻止细胞通过g1/s期转化“关卡”的作用，从而抑制细胞的增殖[9]。文献关于恶性肿瘤中survivin和p27kip1相关性的报道极少。李鹏等[10]发现在胃腺癌组织中survivin的阳性表达率和p27kip1的阳性表达率呈负相关；罗仪等[11]研究发现宫颈癌中，有盆腔淋巴结转移、脉管和（或）间质侵袭患者的survivin阳性表达量高，而p27kip1的表达量与之相反。而erkanli等[12]报道在子宫内膜癌中survivin和p27kip1的表达无相关性。文献未见有关于结肠癌中survivin与p27kip1相关性的研究报道。本研究发现结肠癌中survivin与p27kip1表达无相关性，提示两者可能通过不同的途径参与结肠癌的发生发展。survivin与p27kip1在结肠癌中的确切关系有待进一步研究。
